CHADIS Ages & Stages Questionnaires®
We enhance our pediatric care through the use of CHADIS Ages & Stages Questionnaires®, a medical tool used to screen the development of children beginning at 6 months. Through the parent-reporting online questionnaire, specific information is gathered for your child in areas of development including Communication, Gross Motor, Fine Motor, Problem Solving and Personal-Social. Questionnaire results provide Dr. Grace with instant access to valuable clinical data and resources to aid in the diagnosis and management of health, emotional and behavioral issues.
